WHAT IS INVOKANA?

Invokana is a sodium-glucose transporter-2 (SGLT2) inhibitor used to treat Type 2 diabetes.

SGLT2s are a new category of diabetes drugs introduced in 2013. Other SGLT inhibitors on the market include:

Brand Name Active Ingredients Manufacturer
Invokana Canagliflozin Johnson & Johnson
Invokamet Canagliflozin & Metformin Johnson & Johnson
Farxiga Dapagliflozin AstraZenica
XigduoXR Dapagliflozin & Metformin extended release AstraZenica
Jardiance Empagliflozin Lilly & Boehringer-Ingelheim
Glyxambi Empagliflozin & Linagliptin Lilly & Boehringer-Ingelheim

WHY IS IT PRESCRIBED, AND HOW DOES IT WORK?

Invokana is a once-daily pill that works to lower blood sugar in patients with Type 2 diabetes.

Invokana and other SGLT2s triggers the kidneys to release blood glucose into the urine, thereby lowering blood sugar levels.

INVOKANA AND KETOACIDOSIS

The FDA issued warnings in May of 2015 that Invokana and other SGLT2s may lead to ketoacidosis.

Ketoacidosis is a condition where too much acid (ketones) is produced in the blood.

Warning symptoms of ketoacidosis include nausea, difficulty breathing, vomiting, confusion, abdominal pain and unusual fatigue or sleepiness.

Ketoacidosis can lead to diabetic coma requiring extensive hospitalization, and even death.

INVOKANA AND RISK OF BONE FRACTURES

On September 10, 2015 the FDA required manufacturers to add warnings to the Invokana and Invokamet (canagliflozin) labels about:

  • An increased risk of bone fracture, occurring as early as 12 weeks after treatment initiation, observed in patients using canagliflozin.
  • Canagliflozin has also been linked to decreases in bone mineral density at the hip and lumbar spine.

The FDA continues to evaluate the other SGLT2 inhibitor class drugs (including dapagliflozin (Farxiga, Xigduo XR) and empagliflozin (Jardiance, Glyxambi, Synjardy) for increased risk of bone fractures.
